Members of the Alliance Board were delighted to be able to present their vision at a regional meeting of the national NHS Executive.
The session with the Board was part of series of items during a day-long event held at Harplands Hospitals on 7th December 2017, managed and hosted by North Staffordshire Combined Healthcare NHS Trust on behalf of the Together We’re Better STP.
During the hour-long session, the Board showcased the work of the Meir Hub – winner of the National Positive Practice In Mental Health Award for Health and Social Care 2017 – improving the health and care offer to people with complex lives requiring support in the community, supporting the Primary Healthcare and Social Care system, reducing attendance at Hospital, and reducing costs and creating efficiencies for both health and social care.
The Board also updated the NHS Executive on the development of the Newcastle Extensivist Service, showing the potential for partnership working by delivering a consistent and coherent approach to elderly care in Newcastle-under-Lyme, delivering a consistent approach to the assessment, identification and coding of frailty at practice level, and developing a whole population approach using practice based Elderly Care Facilitators (ECF) for proactive identification of needs.
Simon Stevens NHS Chief Executive and Ian Dodge National Director for Strategy and Innovation were highly interested in the plans and vision being developed by the Alliance and advised the Board in particular to focus on continuing to develop a scalable operational model to enable the Alliance to grow and expand at pace.
